After registering my LLC with the state department, my name does not appear on file, how can I fix this?

I registered my LLC with the state a couple of months ago, but neither my nor my partner's name appear on the doentation we received back acknowledging our establishment. How can I fix this and get our names to show up? We need to set up a bank account, but can't without proof that we are the ones who own the LLC. We're in Pennsylvania, if that makes a difference.
